Directors’ & Officers’ Policy - Claims Reporting Procedures
Please report all Directors’ and Officers’ Liability Claims To:
Axis Reinsurance Company (Canadian Branch), Office of the Chief Agent
In order to report a claim:
Download and complete the Axis claims form
Any letter or documents received by you from the party making a claim against you and All legal papers served on you; and
A copy of your entire file relating to the subject matter of the claim.

General Tips for All Types of Claims
Do not admit liability.
Do not offer compensation to a claimant.
Be sure to record the claimant's name, address, phone number information.
Write down the names, addresses and phone numbers of all witnesses to the incident, including staff.
Do everything you can to minimize or prevent further loss, to the best of your ability.
Make notes of the incident as soon as you can after it occurs. Record details such as the time of day, weather conditions and anything else which might be relevant. Maps and diagrams are often easier.
If you receive correspondence from a claimant, do not respond. Instead forward to the insurer.
Be sure to quote the Master Policy Number and your Certificate Number.
The policy requires that you pay a certain amount for each and every claim as indicated in your Certificate of Insurance.
Renewals and Policy Changes
All renewals, requests to change policy limits and other changes affecting your Certificate of Insurance will be coordinated by Aon.
